You can find the free version in the wordpress repository here: https://wordpress.org/plugins/add-fields-to-checkout-page-woocommerce/ My guess is, it's a copy of the original ThemeHigh plugin. They just changed the author and plugin name.

The premium name is: WooCommerce Checkout & Register form Editor and this is the official homepage: https://www.themelocation.com/woocommerce-checkout-register-form-editor/

I have uploaded it for you BUT there is a problem (as usual).
The plugin is packed with FREEMIUM so we need someone who can null it. @Babak, @TassieNZ, @wmswina ?
