How to add Popup in divi?

Hi,

I know divi had a great community but i think i will get better answer from here. Do any 3rd party plugins available for adding popup like elementor pro. I know bloom optin but again the fields are managed by email provider in bloom.

My requirement is i have schedule appointment button where a user click a popup should come with Name, Email, se*, Type of Disease and Date field with Message also. This should come to email of the configured email. not to mail chimp or sendinblue.
